We are welcoming new patients from all around the Tameside Area, please register online or pop in to see us for the forms .... Our new email address is gmicb-tameside.st.andrewshouse@nhs.net
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be confirmed from your address so it would help if you could provide proof by way of a recent (last 3 months) utility bill. (not a mobile phone bill)
You can either come into the surgery to collect a form or fill out the NHS New Patient form, by clicking on the tiles below.
To complete your registration we may ask you to provide 2 documents to prove your identity.
When you register, it’s helpful to have your NHS number. This is your unique ID number linking you to your medical records. You can use the NHS website or find your NHS number here.
You can fill out the online form by clicking the purple tile below
OR download the paper forms yourself via this link – https://gp-registration.nhs.uk/register-with-a-gp-surgery-paper-form
As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.
To requests proxy access, please collect a proxy access form from reception from 8am to 6.30pm. Or you can download our PDF version on the coloured tiles below.
Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.
The NHS website has information about using linked profiles to access services for someone else.
From 1st April 2015, practices have to divide all patients (including children) with a named accountable GP.
The named GP is responsible for the coordination and delivery of all services, to each of their patients.
Patients do not need to see their named GP when they book an appointment with the practice.
Patients can choose to see any GP or nurse in the practice.
You have a right to choose a GP within the practice this may mean you will have to wait longer for an appointment.
You have a right to an appointment, regardless if you have seen the GP within the last 3 years (12 months if over 75).
